Fill out the form below & SUBMIT when you are done. MRMCA will evaluate and approve all submittals & send all Certificates.

 

2024 Maryland Concrete Conference PDH Request Form:

  1. Fill out your personal information.
  2. Check the appropriate Sessions you attended during the 24th Maryland Concrete Conference and / or Maryland Women in Concrete Forum.
  3. SUBMIT the form. It will be submitted to Mindy Green at MRMCA for approval.

After review and approval, you will be sent a PDH Certificate for each approved Session you attended.

Note: This is for PDHs only. If you need other CEUs or AIAs, there is a separate (live) form that you will fill out, sign, and return to the Registration Table before you leave the Concrete Conference.
